This form is required for all participants attending Encounter 25: Young Adults Camp. It must be completed and submitted before the event and will be retained by the event leaders for 90 days after the camp. The form includes: (1) Participant and camp information, (2) Permission and release of liability, (3) Medical authorization and health details, and (4) Behavior agreement and Code of Conduct.

Permission and Release of Liability

If I am 18 years of age or older, I agree to participate in the event listed. If I am the parent or legal guardian of the participant named above, I give permission for my son/daughter to participate in the event. Although M.A.E.C. and its representatives will use the utmost precaution in safeguarding the health of the participant and preventing accidents, I release them from any liability in case of illness or injury as a result of this activity. Furthermore, I release the owner and driver of the vehicle transporting the participant to and from the event from any liability in case of illness or injury.

Medical Emergency Authorization and Health Information

In case of sudden illness or an accident requiring immediate treatment or surgery while participating in this activity, I authorize the M.A.E.C. Minister(s) to take whatever action is deemed necessary to protect the health and physical well-being of the participant named above. This includes granting permission to any physician(s) and/or surgeon(s) selected by the M.A.E.C. Minister(s) to perform medical or surgical procedures, examinations, or tests necessary for the participant’s care. Every reasonable effort will be made to contact the participant’s emergency contact—or in the case of a minor, their parent(s) or guardian(s)—in the event of an emergency.

Health Information

The following information is requested to assist M.A.E.C. Minister(s) in safeguarding the well-being of the participant during the event. Please list any relevant health conditions, including but not limited to allergies, disabilities, diabetes, asthma, or medications currently required.

Behavior Agreement and M.A.E.C. Code of Conduct

Behavior Agreement

The M.A.E.C. Code of Conduct outlined below is in effect for all activities involving Medhane-Alem Evangelical Church. It applies to all participants in M.A.E.C. activities, with participants defined as all registered youth and adults, and all other individuals who take part and/or attend M.A.E.C. events.

Participants who fail to adhere to the M.A.E.C. Code of Conduct may be subject to a range of disciplinary actions. Immediate corrective action will be taken to ensure the safety and welfare of all participants at the event. Additional disciplinary actions may be taken upon further investigation of the infraction or incident.

If an individual continually disrupts the group or engages in illegal behavior, he or she will be given an opportunity to discuss the problem with the M.A.E.C. Minister(s) before more drastic action is taken. If, after discussion, the behavior continues, or in the opinion of the M.A.E.C. Minister(s) it would be detrimental for the individual to continue with the group, he or she will be sent home at the participant’s expense.

M.A.E.C. Code of Conduct

  1. The health, safety, and welfare of others must be respected at all times.
  2. Appropriate language and behavior are expected at all times. Profanity, foul or abusive language, inflammatory statements, derogatory comments, or physical altercations toward any group or individual are not permitted.
  3. Participants are expected to be present and participating at all scheduled program activities. Participants are required to wear nametags when dispensed.
  4. All participants are expected to be on the site of the event at all times. Unauthorized use of vehicles during an event is prohibited.
  5. Participants are responsible for following instructions of all M.A.E.C. Ministers and volunteer adult leaders.
  6. All behavior or language of a sexual nature at M.A.E.C. events is inappropriate and unacceptable. Dignified and respectable behavior is expected at all times.
  7. Dorm facilities are absolutely off-limits to members of the opposite sex and curfew hours must be strictly followed.
  8. Behavior during unscheduled free time is subject to the supervision of M.A.E.C. Ministers and volunteer adult leaders.
  9. All participants must meet dress code standards set for the event (i.e., no sexually suggestive, culturally insensitive, tobacco or alcohol industry-sponsored shirts, inappropriately cut shirts, shorts, pants, or skirts, etc.).
  10. Possession, distribution, or use of alcoholic beverages, cigarettes, or illegal drugs is prohibited. Prescription drugs and over-the-counter medications may be dispensed by adult leaders only with written authorization provided by the parent/guardian on the Medhane-Alem Evangelical Church Youth Event Permission Form.
  11. Care and respect for property, personal and institutional, is expected at all times. Theft, possession of missing property, or damage to property is prohibited.
  12. Unauthorized possession, distribution, or use of weapons, ammunition, or fireworks is prohibited.
  13. Honesty is expected at all times from M.A.E.C. event participant. Dishonesty, cheating, plagiarism, and forgery are inappropriate actions.
I have read and agree to abide by the Behavior Agreement and M.A.E.C. Code of Conduct above. If I am the parent or guardian of the participant, I have also reviewed and discussed it with my son/daughter. I understand and accept that failure to follow the agreement may result in the participant being sent home at my expense.
